Upon submission, I authorize Walter's Pet Styles to charge my card on file for 12 appointments with 25% off plus tax. If I prefer to pay in person, I will inform Walter's Pet Styles on this form and make payment on the date of this submission. I will schedule to redeem my first pre-paid service within 30 days of purchase. The subsequent 11 remaining services will be auto-booked according to my preferences above, once per month until complete. Unused services will not roll-over into future months, be split between pets, and payment is non-refundable. This is a 'use it or lose it' deal. I understand while priority will be granted for rescheduling within the appointment month, availability may be limited and I may need to be flexible. This deal can not be combined with other specials or promotions (ex: Sunday & Monday Bath & Basics Specials). I agree that it is still my responsibility to maintain my pet between appointments and understand de-matting fees may apply (as is standard policy) if my pet is presented in excessively tangled condition.
